Areola restoration tattooing, also called nipple-areola complex (NAC) tattooing or medical tattooing, uses pigment and shading to recreate the realistic appearance of the areola and/or nipple. It’s precise, detailed work, and in the right hands it can be extraordinary.

For many people, whether they’re a breast cancer survivor, a trans man who just had top surgery, or a nonbinary person navigating their own relationship with their chest, this tattoo is the final step in a long and difficult journey.
Top surgery, which is chest masculinization surgery for trans men and nonbinary people, often involves the removal or repositioning of the nipples and areolas. Depending on the surgical technique used, grafted nipples may heal lighter or with less definition than before. Some people lose sensation in the area entirely. Others opt to have their nipples removed and want to recreate the look with tattooing alone.

Before any ink touches your skin, we talk in person. We want to understand your surgical history, what you’re hoping for, and what the healed result should look like. Size, shape, color, symmetry, placement: all of it is discussed and customized to you specifically. There is no one-size-fits-all here, and that’s especially true for clients coming from different surgical backgrounds.
Areola tattooing is delicate, detailed work. It often involves layering shades to create a three-dimensional appearance, particularly when recreating natural texture and color variation. For clients without reconstructed or grafted nipples, artists can also create the illusion of projection using shadow and highlight techniques.
Sessions typically last two to four hours depending on complexity. Scar tissue, whether from mastectomy, top surgery, or another procedure, behaves differently than unscarred skin. A skilled artist knows how to work with it, not against it.
We’ll send you home with clear aftercare instructions. Healing typically takes two to four weeks. In all cases a touch-up session is needed once the skin has fully healed, just to ensure we’re all happy with the outcome.

Does areola tattooing hurt?
Scar tissue often has reduced sensation, so many clients find this to be less uncomfortable than traditional tattooing. Everyone’s experience is different and we’ll go at whatever pace you need.
How long do I need to wait after surgery?
Generally, surgeons recommend waiting at least one year after your final procedure before tattooing the area. This applies whether you’ve had a mastectomy, top surgery, or another chest procedure. We’ll confirm this with you during consultation.
I had top surgery and my nipples were grafted. Can you still work on them?
Yes. Grafted nipples are something we have experience with. The skin heals and behaves differently than native tissue, and we factor that into how we approach the work. If you opted for nipple removal and want the areola recreated without a physical nipple, we can do that too.
Do I need a referral from my surgeon?
No referral needed. If your surgeon has specific guidance about timing or aftercare, we’d love to know, but it’s not a requirement to book.
Can you match the color of my other nipple?
Yes. Color matching is a priority in this work. We’ll discuss it in detail during your consultation and make sure we get it right.
Is this covered by insurance?
Areola tattooing following mastectomy may be covered under the Women’s Health and Cancer Rights Act (WHCRA), so it’s worth checking with your insurance provider. Coverage for gender-affirming procedures varies widely. Either way, this service costs you nothing at Inkvictus. $0, always.
